SPECIAL INCIDENT REPORT

This form should be used to report a Special Incident not covered by Team Official or Player dismissal reports including but not limited to, further misconduct following a dismissal.
This report may be filed by any appointed Official, Referee Assessor or Supervisor of Officials.
The Referee must submit this report within 24 - 48 hours of the completion of the game the incident occurred.
Offences against match officials must be referred to the Soccer Nova Scotia Discipline Committee.
